Legal Data Entry Assistant for Risk Management and Incident Reporting Logs

Objective:
Accurately record and maintain organizational risk management data and incident reporting logs in a structured spreadsheet or compliance database. Ensure information is properly categorized, up to date, and easily accessible for audits and internal reviews.

Scope of Work:

  1. Collect incident reports, risk assessments, and compliance notes from provided source materials.

  2. Enter details into the designated spreadsheet or risk management tracking system.

  3. Categorize incidents by type, severity, and location.

  4. Verify accuracy of data against original reports.

  5. Flag incomplete, unclear, or inconsistent information for follow-up.

  6. Prepare a Word report summarizing data entry progress, flagged issues, and recommendations for corrective actions.

Required Data Fields:

  • Incident ID / Reference Number

  • Date of Incident

  • Location of Incident

  • Individuals Involved / Reported By

  • Type of Incident (safety, compliance, fraud, workplace, etc.)

  • Severity Level (low, medium, high, critical)

  • Description of Incident

  • Action Taken / Resolution

  • Responsible Department / Officer

  • Follow-up Requirements or Recommendations

Deliverables:

  • Completed Excel spreadsheet with incident and risk management records.

  • Word report highlighting updates, issues encountered, and suggestions for improvement.
